Our Journey to Becoming a CCBHC

As a result, of ongoing behavioral health work and a physical health pilot, Centers for Family Development, Inc. (CFDI) was selected for the Certified Behavioral Health Grant, a concept championed and implemented nationally by Senator Debbie Stabenow, to expand integrated care into our targeted zip codes by adding a nurse practitioner, a medical assistant, and a care coordinator.

Services Shaped by Community Feedback

In 2024, CFDI’s CCBHC conducted a community needs assessment with Detroit residents. Most engaged were Detroiters from CFDI’s 3 focal point neighborhoods for the past 20 years, Denby, Osborn, and Cody Rouge. The community needs assessment was important to CFDI to better understand the areas of health challenges from residents’ perspectives. This included gathering surveys, organizing focus groups, and meeting with individuals, families, and community leaders to discuss behavioral health needs and the factors that affect overall health, such as economic and social conditions. The information collected helped set goals for the CCBHC. Detroit residents’ and stakeholders’ key areas of focus include addressing barriers to care, such as transportation and access to reliable health information, hiring staff who are responsive to the community, and building strong partnerships to improve care coordination.



Sliding Scale Program

The Hope and Healing Clinic has people who can help you apply for insurance. If you don’t make a lot of money, don’t have insurance, or your insurance costs too much, the Clinic has a discount program to help. The Sliding Scale program gives lower prices based on how much money you earn and how big your family is. Please call and ask to talk to the Nurse Manager to learn more.
