Care Management Organization (CMO)

The CFDI's CMO program provides, and monitors identified services to youth and families that have come within the jurisdiction of the Wayne County Department of Juvenile and Youth Services (JYS). This program provides a variety of residential and community-based resources and services to youth and their families. In addition to providing direct services and accessing resources, the CMO Case Manager works closely with the youth and their family to ensure that the youth has the resources, skills, and opportunities to achieve success beyond release from probation services.
